Assisting Hands Home Care – South Jacksonville is proud to partner with PocketRN through the CMS GUIDE program to bring up to 80 hours of paid home care and virtual nursing support at no cost to qualifying families living with dementia.
What’s Included?
- A dedicated dementia-trained nurse for phone or video check-ins
- 24/7 access to a virtual care team
- Care coordination with your loved one’s doctors, specialists & services
- Medication review & chronic condition support
- Education & 1:1 caregiver coaching
- Referrals to local resources and support systems
Who Is Eligible?
To qualify, the individual must:
- Have moderate to severe dementia (no official diagnosis required)
- Be enrolled in Traditional Medicare (Parts A & B)
- Live at home or in an assisted living facility
- Not be on hospice or living in skilled nursing care
- Have a family or unpaid caregiver (does not need to live with them)
VA patients, dually eligible Medicare/Medicaid individuals, and those receiving memory or palliative care may also qualify!
